Psychosocial Rehabilitation (PSR)

Psychosocial Rehabilitation (PSR) is a therapeutic, multidisciplinary approach designed to help individuals with mental health disorders or disabilities regain lost skills, improve social functioning, and live more independently within their communities. Creative Directions PSR programs are a structured 13 guided (by PSR Specialists) sessions program that focuses on developing life skills, social skills, vocational readiness, and coping strategies tailored to each person’s unique needs and goals. The goal is to support recovery, promote resilience, and enhance quality of life by empowering individuals to function successfully in work, social, educational, and everyday settings. Services are person-centered, recovery-focused, and delivered in community environments with a strong emphasis on hope, empowerment, and social inclusion.

Eligibility for Creative Directions PSR program
- A primary diagnosis of a mental health disorder that results in significant functional impairment affecting basic independent living skills.
- Presence of difficulties in at least two areas such as maintaining interpersonal relationships, managing daily living activities (personal hygiene, nutrition, finances), or using community resources.
- History of long-term or repeated psychiatric hospitalizations or significant impairment that requires ongoing intensive rehabilitation services to maintain community living.
- The individual must be capable of engaging in the program and not be at imminent risk of harm to self or others, with less intensive services judged inadequate to address their rehabilitation needs.
- Informed consent to participate and medical necessity documented by a licensed healthcare provider.
- Has a valid active Medicaid Insurance.
